Thank you for your interest in working with Whosurcaddie.com to provide a centralized, efficient, and professional caddie service that caters to the needs of golfers during the course of a game. The following list must be completed and submitted prior to beginning your service.
Completed Caddie Bio Form
- Picture of Yourself (preferably in digital form sent to This e-mail address is being protected from spambots. You need JavaScript enabled to view it )
- Signed Independent Contractor Agreement (download here)
- Completed & Signed Income Tax Form W-9 (complete instructions to the form can be found at www.irs.gov)
- Annual Membership Fee of $120 (renewable on service start date)
- Paid out in the first six loops @ $20 per loop, or
- Paid in advance with a $20 discount, please enclose a check for $100.

Once we receive the completed list above, you will be informed of your Log In Name and Password prior to your start date, which you will use to get on our website, www.whosurcaddie.com, and verify your Caddie Bio information and set your own calendar. When that process is completed, you will be ready to caddie!
